Voldemort wins in the first war (prophecy doesn't exist in this universe)
Creatures are given better rights and muggleborns are integrated into magical society after their first burst of accidental magic. If they are found in a bad situation they are placed in a magical orphanage where they can be adopted by magical families. If they aren't their muggle parents are made to sign secrecy contracts.
Hermione is found neglected by her muggle parents and is blood adopted by Sirius and Remus (who weren't killed because they never joined the war effort in order to keep their goddaughter, Lily and James safe).
Fem! Harry = Harrietta Liliana Potter
Hermione's new name = Hermione Cassiopeia Black (her appearance doesn't really change except instead of brown her eyes become the Blacks' silver)
Voldemort has a son (also named Tom) who he sends to Durmstrang.
Hermione and Harri aged up so they are 18 and 17 in 1994, respectively.
Triwizard tournament still happens.
Overview of what I want to happen:
Harri and 'Mione are known for constantly researching and experimenting and therefore aren't present when the Durmstrang and Beuxbaton schools arrive at Hogwarts l. Voldemort visits Hogwarts seeing as his heir is there and together with Snape walk around the school in order to discuss the going one of the tournament and the school. While doing so they are interrupted by a student running down the hall yelling for Professor Snape. Snape snaps at the student to tell him what he wants because obviously he is in the middle of something very important. The student sees Voldemort and Tom, pales, and stutters out that Another Male student had tried to get handy with Potter and- but seeing as it's well known that Harri doesn't take that sort of thing laying down he immediately starts towards where the student said the incident was taking place with Voldemort and Tom following curiously behind. When they arrive they see Hermione half-heartedly telling Harri to stop cursing, hexing, jinxing, and cursing out the male student who had groped her. Tom immediately becomes infatuated with Harri and spends the year trying woo Harri and convince her to accept his courting.
There would also be a Hermione/Viktor side pairing.
