Clinical History:
Mr. Barnes, an enhanced individual due to the Super Soldier Serum, referred by Dr. Stephen Strange, presents with intermittent dizziness, episodes of disorientation, and occasional visual disturbances. Due to Mr. Barnes' vibranium arm, MRI was deemed impractical.
Findings:
A contrast-enhanced CT scan of the brain was performed.
- Location and Size: A sizable, irregularly contoured mass measuring approximately 5.5 cm in diameter is located in the anterior left frontal lobe.
- Enhancement Pattern: The lesion exhibits intense and heterogeneous enhancement post-contrast administration, indicative of an infiltrative nature, typical of glioblastoma.
- Mass Effect: A significant mass effect is noted, causing effacement of the adjacent cortical sulci and displacement of the left lateral ventricle, resulting in a midline shift of 1.7 cm to the right.
- Edema: Extensive peritumoral edema involving the left frontal lobe extends into the adjacent white matter tracts, underscoring the lesion's aggressive behavior.
- Additional Findings: No evidence of acute hemorrhage or hydrocephalus. The remainder of the brain parenchyma appears unremarkable.
Impression:
The imaging findings are highly suggestive of a glioblastoma involving the left frontal lobe. The presence of a large, enhancing mass with surrounding edema and significant mass effect is indicative of an aggressive neoplastic process.
Recommendations:
- Neurosurgical Consultation: Urgent referral to a neurosurgical team for consideration of biopsy or resection, with consideration for the unique challenges posed by Mr. Barnes' enhanced physiology from the Super Soldier Serum and vibranium arm.
- Alternative Imaging Consideration: Given the impracticality of MRI, explore alternative imaging methods or adaptations for comprehensive evaluation of the lesion's characteristics and extent.
- Oncology Referral: Once a definitive diagnosis is established, immediate referral to oncology is advised for the formulation of a comprehensive treatment plan, potentially encompassing surgical intervention, radiotherapy, and systemic therapy.
