The geography of the world is basically the same. One of the few differences is that there are not 50 states in America but 51, including the new "state" of Panem (Hunger Games reference). Back during the War of 1812, when the British Canadians plotted to burn down the White House, a city in British Columbia seceded from Canada to join the Union. This city warned the Americans of the attack and because of their courage and bravery, the President and his staff were able to evacuate in time enough and plan a counterattack. At the end of the War, Panem became a state of the Union.
There were fourteen districts in Panem. Those districts became the cities of Panem once it became a state. The fourteenth district changed into what is known as Capital city. During the Civil War, chaos ensued all across the country, and by the war's end, District 13 no longer existed.
Another thing that you need to know is that there aren't seven continents in the world. There are eight, including Westeros which is located in the Pacific Ocean. Essosi cities such as Vaes Dothrak, Astapor, Meereen, Yunkai, Pentos, Braavos, Asshai, the Rhoyne, Quarth, Lys, and Volantis are located in the Middle East.
The modernly named country-states within Westeros are as follows:
• Norwood (a.k.a. the North)
• Riverland (a.k.a. the Riverlands)
• Valemont (a.k.a. the Vale)
• New Westminster (a.k.a. the Westerlands)
• Reach Valley (a.k.a. the Reach)
• Iron Islands (I don't even have to explain)
• Dorne (^^^^)
• Stagfeld (a.k.a. the Stormlands)
• Fiora (a.k.a. the Crownlands/Dragonstone)
