Hermione Has Multiple Avatars Fairy Avatars, and one Human-Fairy Avatar (a Witch)
First is her true Fairy Form, with expanded wings (colour white), with flowers rising through her skin, dancing in the moonlight near the river edge.
Then, it's her Travelling Form Avatar. A form Fairies usually take outside fairyland, by changing their wings into a more practical shape. They hide the flowers attached to their skins, or diamonds or anything else and instead wear on their heads an identifier of the clan, or family they belong to. In Hermione's case, she is a Forest Flower Fairy. The Moonlight ( the white shades in her hair is also not reflected anymore), for the fair only reflects the Fairy Moon. In the Magical Land, each territory comes with a different shining type of Moonlight.
And Her Third Fairy Avatar form is the form without Wings, in which she appears when she is doing day-to-day tasks in which the wings would be inconvenient. It is also the form where she meets Severus for the second time in.
Severus is a Fae. That is the form he takes usually.
